Patient Participation Group

We have a patient participation group at the practice that meets every quarter. If you would like to join the group please download the application form or obtain a copy from reception.

If you are unable to attend quarterly meetings and would still like to be part of the group you may be a “virtual” member and provide your views and receive correspondence electronically. All we need is your consent to email you information. To do this please click on the link in the image opposite to complete the online form.
