You can use this form to submit non-urgent medical requests that we be responded to within three working days

Both medical and admin queries can be submitted through this service.

Please consider the following before completing a form:

1. This service is for non-urgent requests- If you have a same day or urgent request, please contact the practice by phone on 01226 216000
2. Any prescriptions ordered through this service will take a minimum of 48 hours. If you need a same day prescription, please contact the practice by phone.
